You didn't ask about why fat people stay fat. You asked why thin people aren't fat.
there is no universal answer. It is a combination of reasons that vary from person to person. Someone may be thin primarily because of their metabolism or because of the amount of exercise they do. Regardless it's never exclusively one reason and some people require more discipline than others.
It's also important to note that just because someone is thin doesn't mean their cholesterol level can't be as bad or worse than someone who is overweight.

Anyone who consumes fewer calories than they burn will lose body fat, then muscle mass. Anyone who consumes the same number of calories as they burn will neither gain nor lose weight. Anyone who consumes more calories than they burn will gain weight.
